After receiving orders from a Physician to initiate care, our highly trained and skilled professional staff performs a thorough evaluation. We then establish a Plan of Care (POC) based on this evaluation and strictly adhere to all physicians’ orders. This POC is carried out by all staff members involved in the patient’s care.

A Registered Nurse (RN) Supervisor closely monitors each patient’s progress and communicates with the physician when necessary. Once all goals that were established in the POC are met, the physician is informed and the patient is discharged from our agency.

If goals are not met within the time frame predicted in the POC, then the RN Supervisor requests authorization from the physician to re-evaluate the patient and extend the frequency of services.
